History and Mission Statement
San Diego Independent ACO,
LLC ( SDINDYACO) is a newly formed Independent ACO comprised
of Primary Care Providers PCP, Family Physicians and Internists with a proud
history of serving local San Diego residents just like you for the past 20+
years.
Our mission is to give our Medicare
Fee-For-Service beneficiaries personalized quality care that is
coordinated and tailored to their individual needs.
So what is an ACO ?
An Accountable Care
Organization, ACO is a group of doctors and other health care providers working
together with Medicare to give you better service and care. The goal of an ACO is for your doctors to
communicate closely with your other health care providers to deliver
high-quality care and meet your unique individual needs and preferences. The ACO may be rewarded for providing you
with high quality, more coordinated care.
What can an ACO do for me?
We believe that Medicare
Fee-For-Service beneficiaries with a personal Primary Care Physician
coordinating their health care among any and all specialists and all area hospitals
receive better care more efficiently with less time consuming duplication.
· You benefit because your doctors will be part of a better coordinated team.
· You may not have to fill out as many medical forms that ask for the same information.

Each of your doctors will not only know about
the health issues they’ve treated, they will have a more complete picture of
your health through talking with your other doctors.
· Our doctors work with other health care providers and area hospitals who voluntarily work together with Medicare to give you high quality service and care at the right time in the right setting.
Can I Still See Any Doctor or go to any Hospital? Yes.
Your Medicare benefits are
not changing, and you will still receive your benefits through Original
Medicare. This isn’t a Medicare
Advantage plan or an HMO plan. You still have the right to use any doctor or
hospital who accepts Medicare, at any time.
Your SDIACO doctor may continue to recommend that you see particular
doctors for your specific health needs, but it’s always your choice about what
doctors you use or hospitals you visit.
Who are the doctors that belong to San Diego Independent Accountable Care
Organization?
All of our providers are Primary
Care physicians with years of experience serving Medicare patients on
a conventional fee for service basis and many also have experience serving
seniors on Medicare Advantage programs.
A complete list of all of our primary care physician providers and
information concerning our governance appears below.
How do I Participate?
If you are not part of a
Medicare advantage HMO plan and you have conventional fee-for-service Medicare
you may have already received a letter inviting you to participate in an ACO.
You may have also been
invited to participate after visiting a Medicare physician who participates in
the ACO (listed above).
Indeed if you have not been notified and wish
to be part of an ACO you can contact any one of the above physician offices and
make arrangements to participate in the Medicare ACO program. You can also
visit www.medicare.gov/acos.html
or call
1-800-MEDICARE and tell the operator you are asking about ACOs (TTY users
should call 1-877-486-2048).

Composition of ACO:
Independent San
Diego ACO, Doing business as San Diego Independent ACO with the
acronym of SDIACO or SDINDYACO is a LLC Limited Liability
Corporation. It consists solely of ACO professionals providing coordinated
services to Medicare Beneficiaries in San Diego California.
SDIACO operates independent of any hospital facility
affording it's PCPs the opportunity to admit patients
across all San Diego area hospitals.
